All work, with the exception of records release, will remain confidential in nature. By agreeing to the terms, you are giving consent to reveal limited-scope progress reports and updates on your health to Martha’s professional advisors and colleagues, for the sole purpose of helping to support your health outcomes. You have a right to access your own client records. All records remain confidential, with the exception noted above, unless you specifically authorize release in writing or otherwise provided by law.

ASSESSMENT OF PROGRESS:
Following your session you will be sent a progress report/questionaire to fill out.

You have a right to complete and current information regarding the assessment methods. When sessions are complete, you have a right to coordinated transfer if there is a change in provider of services.

You give Martha G. Blessing permission to use your words (you will NOT be personally identified and will remain anonymous).
